Overview
- Deliver person-centered care management services, ensuring compliance with state regulations and organizational standards.
- Conduct quarterly in-person member visits within a reasonable geographic radius.
- Coordinate services and benefits navigation to support members' health and well-being.
- Develop and maintain individualized Life Plans for members, collaborating with families and service teams.
- Monitor adherence to care and treatment recommendations, ensuring timely access to healthcare services.
- Utilize Health Information Technology systems to document services and maintain compliance.
- Support transitions between care levels, providing timely follow-up after critical events.
- Engage families with support groups and resources to enhance member satisfaction.
Key Responsibilities & Duties
- Complete comprehensive assessments covering medical, mental health, and social service needs.
- Develop and manage caseloads, conducting required face-to-face visits with members.
- Coordinate interdisciplinary teams to implement clinical recommendations and care plans.
- Provide transitional care support, ensuring smooth transitions between care levels.
- Identify and manage referrals to community and social support services.
- Maintain accurate documentation in Electronic Health Records, adhering to HIPAA regulations.
- Assist members with benefit programs and personal planning needs.
- Report any concerns related to abuse, neglect, or maltreatment promptly.
Job Requirements
- Associate of Applied Science (AAS) degree or higher in a relevant field.
- Minimum of 1 year of relevant case management experience; preferred 2 years.
- Proficiency in Microsoft Suite and Health Information Technology systems.
- Strong interpersonal, conflict-management, and communication skills.
- Ability to prioritize tasks, analyze situations, and act effectively under pressure.
- Knowledge of benefit programs such as SNAP, Medicaid, and SSI.
- Experience with interdisciplinary team collaboration and care plan development.
