Overview

  • Provide occupational therapy services to patients in a home health setting, ensuring personalized and effective care.

Key Responsibilities & Duties

  • Conduct initial evaluations and re-evaluations to assess patient needs and progress.
  • Develop and implement occupation-centered treatment plans to enhance patient functionality.

Job Requirements

  • Experience with OASIS and Medicare documentation preferred.
  • Strong time management and independent practice skills essential.
  • Reliable transportation for travel to patient locations.
  • High School Diploma or GED required; further education in occupational therapy preferred.
