Job Description

Overview

  • Serve as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
  • Provide comprehensive care to underserved populations with complex medical needs.
  • Conduct well-woman exams, preventive screenings, and chronic disease management.
  • Collaborate with interdisciplinary teams to ensure high-quality patient outcomes.
  • Engage in both clinic and in-home patient visits for holistic care delivery.
  • Utilize evidence-based guidelines to address primary care and gynecological concerns.
  • Support patients through pregnancy, menopause, and chronic condition management.
  • Maintain accurate documentation within the Electronic Medical Record (EMR) system.

Key Responsibilities & Duties

  • Perform comprehensive history and physical exams for risk adjustment and assessments.
  • Provide full-spectrum contraceptive counseling and services, including IUD insertions.
  • Diagnose and treat sexually transmitted infections and provide sexual health counseling.
  • Develop individualized care plans for chronic conditions like hypertension and diabetes.
  • Collaborate with specialists for seamless prenatal and specialty care coordination.
  • Screen for social determinants of health, including domestic violence and substance use.
  • Engage in Tele-Health communications for accessible patient care delivery.
  • Ensure compliance with New York State laws and clinical practice guidelines.

Job Requirements

  • Master of Science in Nursing with FNP certification.
  • Current RN licensure and DEA registration in New York State.
  • 3–5 years of experience in Women's Health and Primary Care.
  • Proficiency in phlebotomy, immunizations, and office-based procedures.
  • Experience working with underserved populations facing healthcare barriers.
  • Fluency in an additional language is highly preferred.
  • Strong clinical judgment and ability to collaborate effectively with teams.
