Overview

  • Support individuals facing housing insecurity through outreach and case management services in a dynamic urban environment.
  • Collaborate with a dedicated team to connect clients with essential social services and community resources.

Key Responsibilities & Duties

  • Conduct proactive outreach to individuals experiencing homelessness, building rapport and trust.
  • Provide mobile case management services to connect clients with tailored resources and support.
  • Collaborate with social service providers and community organizations to facilitate resource access.
  • Utilize bilingual skills to enhance communication and outreach effectiveness.
  • Maintain accurate documentation of outreach activities and client interactions.
  • Participate in regular team meetings and training sessions to improve service delivery.
  • Support program goals by addressing housing insecurity through innovative approaches.
  • Engage with the community to promote awareness and foster partnerships for program success.

Job Requirements

  • Bachelor of Science degree in social services or a related field.
  • Minimum of 2 years of experience in case management or community outreach roles.
  • Preferred certifications include Certified Recovery Peer Advocate (CRPA) or Certified Peer Specialist (CPS).
  • Lived experience with homelessness, substance use, or mental health challenges is highly valued.
  • Proficiency in Spanish is strongly preferred for effective communication.
  • Comfortable working on-site and conducting outreach in public spaces.
  • Strong interpersonal skills and ability to build rapport with diverse populations.
  • Availability for a temporary role with potential for permanent placement.
