Key Responsibilities & Duties
- Conduct proactive outreach to individuals experiencing homelessness, building rapport and trust.
- Provide mobile case management services to connect clients with tailored resources and support.
- Collaborate with social service providers and community organizations to facilitate resource access.
- Utilize bilingual skills to enhance communication and outreach effectiveness.
- Maintain accurate documentation of outreach activities and client interactions.
- Participate in regular team meetings and training sessions to improve service delivery.
- Support program goals by addressing housing insecurity through innovative approaches.
- Engage with the community to promote awareness and foster partnerships for program success.
Job Requirements
- Bachelor of Science degree in social services or a related field.
- Minimum of 2 years of experience in case management or community outreach roles.
- Preferred certifications include Certified Recovery Peer Advocate (CRPA) or Certified Peer Specialist (CPS).
- Lived experience with homelessness, substance use, or mental health challenges is highly valued.
- Proficiency in Spanish is strongly preferred for effective communication.
- Comfortable working on-site and conducting outreach in public spaces.
- Strong interpersonal skills and ability to build rapport with diverse populations.
- Availability for a temporary role with potential for permanent placement.
