Key Responsibilities & Duties
- Provide care for patients with acute and chronic illnesses, ensuring optimal health outcomes.
- Lead a care team, including medical assistants and care coordinators, in delivering patient-centered services.
- Engage with hospitalists and specialists to coordinate comprehensive patient care.
- Document patient interactions and treatment plans efficiently using advanced systems.
- Participate in clinical meetings and quality management programs to enhance care delivery.
- Supervise and collaborate with Advanced Practice Practitioners (APPs) in patient care.
- Promote evidence-based medicine and lifestyle interventions for improved patient outcomes.
- Support the center's operational goals and assist in covering for other providers as needed.
- Engage in recruiting efforts for patients and healthcare professionals to expand services.
Job Requirements
- Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
- Active State Medical License or eligibility to obtain one within the required timeframe.
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification within 90 days of employment.
- Minimum of 1 year of experience, with 5 years preferred, in primary care or similar settings.
- Proficiency in Microsoft Office Suite and healthcare documentation systems.
- Strong commitment to patient-centered care and mission-driven healthcare delivery.
- Ability to travel locally and nationally for training and recruitment purposes.
- Fluency in English, with additional language skills preferred for specific centers.
