Job Description

Overview

  • Lead the case management department in a hospital setting, ensuring compliance with policies and regulatory standards.
  • Coordinate patient care and discharge planning processes to optimize clinical resource utilization and continuity of care.
  • Develop and implement performance improvement initiatives to enhance departmental operations and outcomes.
  • Collaborate with interdisciplinary teams and external stakeholders to achieve organizational goals.
  • Provide leadership and mentorship to staff, fostering a cohesive and supportive team culture.
  • Adapt quickly to organizational priorities and regulatory requirements, demonstrating sound judgment in decision-making.
  • Ensure adherence to care coordination and discharge planning processes, maintaining timeliness and quality.

Key Responsibilities & Duties

  • Direct and evaluate departmental operations, including staffing, onboarding, and competency development.
  • Allocate resources effectively to meet productivity and quality goals.
  • Oversee case management core functions and practices, ensuring compliance with standards.
  • Collaborate with hospital leadership to achieve established goals and expectations.
  • Monitor departmental performance through data analysis and implement improvement activities.
  • Establish relationships with key stakeholders to ensure alignment and support for initiatives.
  • Promote team engagement and foster a supportive case management culture.
  • Adapt to changes and coordinate efforts across multiple stakeholders effectively.

Job Requirements

  • Bachelor's degree in Nursing, Health Administration, or related field, required.
  • Master's degree in Nursing, Health Administration, or Business Administration, preferred.
  • Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW), required.
  • Minimum 3 years of acute care hospital case management experience, required.
  • Minimum 2 years of case management leadership experience, required.
  • Proven leadership experience in an acute care setting, with a focus on driving performance and patient outcomes.
  • Ability to adapt quickly to organizational priorities and regulatory requirements.
  • Strong communication, presentation, and motivational capabilities.
