Hospice Team Manager

Overview

• Lead and mentor a dedicated nursing team in delivering exceptional hospice and palliative care services.
• Ensure compliance with program philosophy, goals, and policies to maintain high standards of care.
• Manage staff scheduling and assignments in alignment with established staffing ratios and guidelines.
• Empower RN Case Managers to excel in pain and symptom management and patient-centered care.
• Evaluate nursing documentation quality to ensure adherence to policies and regulations.
• Collaborate with Admissions and Outreach teams for community presentations and engagement.
• Support interdisciplinary team efforts to deliver holistic patient and family care.
• Participate in hybrid work arrangements with defined in-office training requirements.

Key Responsibilities & Duties

• Provide leadership and daily guidance to nursing staff, ensuring consistent care delivery.
• Schedule staff assignments in collaboration with the Associate Director of Patient Services.
• Mentor RN Case Managers in pain and symptom management and patient-focused care planning.
• Monitor and evaluate nursing documentation for compliance with policies and regulations.
• Engage in community outreach and presentations to promote hospice services.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain availability for incident follow-ups beyond core working hours.
• Participate in rotating weekend coverage with flexible scheduling options.

Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited institution required.
• Minimum of five years of hospice nursing experience, with supervisory experience preferred.
• Active New York State RN licensure and CHPN certification preferred.
• Proficiency in Word, Outlook, and Windows applications required.
• Ability to supervise and mentor nursing staff effectively.
• Strong interdisciplinary team collaboration skills.
• Availability for hybrid work arrangements and rotating weekend coverage.
