Overview
- Lead and mentor a dedicated nursing team in delivering exceptional hospice and palliative care services.
- Ensure compliance with program philosophy, goals, and policies to maintain high standards of care.
- Manage staff scheduling and assignments in alignment with established staffing ratios and guidelines.
- Empower RN Case Managers to excel in pain and symptom management and patient-centered care.
- Evaluate nursing documentation quality to ensure adherence to policies and regulations.
- Collaborate with Admissions and Outreach teams for community presentations and engagement.
- Support interdisciplinary team efforts to deliver holistic patient and family care.
- Participate in hybrid work arrangements with defined in-office training requirements.
Key Responsibilities & Duties
- Provide leadership and daily guidance to nursing staff, ensuring consistent care delivery.
- Schedule staff assignments in collaboration with the Associate Director of Patient Services.
- Mentor RN Case Managers in pain and symptom management and patient-focused care planning.
- Monitor and evaluate nursing documentation for compliance with policies and regulations.
- Engage in community outreach and presentations to promote hospice services.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Maintain availability for incident follow-ups beyond core working hours.
- Participate in rotating weekend coverage with flexible scheduling options.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution required.
- Minimum of five years of hospice nursing experience, with supervisory experience preferred.
- Active New York State RN licensure and CHPN certification preferred.
- Proficiency in Word, Outlook, and Windows applications required.
- Ability to supervise and mentor nursing staff effectively.
- Strong interdisciplinary team collaboration skills.
