Job Requirements
- Bachelor of Science in Nursing (BSN) and completion of a Nurse Practitioner program.
- Unrestricted Nurse Practitioner license in New York State.
- DEA certification for prescribing medications within scope of practice.
- Eligibility to provide services reimbursable under Medicare and Medicaid.
- Minimum of 2 years of clinical experience, preferably in long-term care.
- Proficiency with electronic health record systems, especially eClinicalWorks.
- Strong understanding of chronic care management practices.
- Excellent communication and teamwork skills.
