Nurse Practitioner

Overview

• Deliver exceptional patient care as a Nurse Practitioner in a supportive clinical environment.
• Perform comprehensive health assessments, including physical exams and medical histories.
• Collaborate with interdisciplinary teams to develop personalized care plans.
• Manage chronic conditions and acute illnesses with evidence-based interventions.
• Order and interpret diagnostic tests to inform treatment decisions.
• Prescribe medications and treatments in accordance with state regulations.
• Educate patients and families on health maintenance and disease prevention.
• Participate in initiatives to enhance care quality and outcomes.
• Maintain accurate and thorough documentation in electronic health records.

Key Responsibilities & Duties

• Conduct physical exams and medication reconciliation for patients.
• Collaborate with healthcare teams to create individualized care strategies.
• Manage acute and chronic conditions with appropriate medical interventions.
• Order and analyze diagnostic tests to support clinical decisions.
• Prescribe medications and treatments following established guidelines.
• Educate patients and families on health improvement and disease prevention.
• Participate in chronic care management and service initiation.
• Contribute to quality improvement projects and interdisciplinary meetings.
• Ensure precise documentation in electronic medical records systems.

Job Requirements

• Bachelor of Science in Nursing (BSN) and completion of a Nurse Practitioner program.
• Unrestricted Nurse Practitioner license in New York State.
• DEA certification for prescribing medications within scope of practice.
• Eligibility to provide services reimbursable under Medicare and Medicaid.
• Minimum of 2 years of clinical experience, preferably in long-term care.
• Proficiency with electronic health record systems, especially eClinicalWorks.
• Strong understanding of chronic care management practices.
• Excellent communication and teamwork skills.
• Willingness to travel to multiple sites within a designated radius.
