Key Responsibilities & Duties

- Provide support and guidance to residents and families regarding care options.

Job Requirements
- Master’s degree in Social Work (MSW) required; LMSW certification preferred.
- Experience in geriatrics, long-term care, or healthcare preferred.
- Proficiency in Point Click Care (PCC) software is advantageous.
- Strong skills in case management and discharge planning.
- Ability to assess resident needs and develop effective care plans.
