Overview

Key Responsibilities & Duties

  • Coordinate resident services and ensure effective communication among care team members.
  • Conduct assessments to identify residents' needs and develop personalized care plans.
  • Facilitate discharge planning and transitions to appropriate care settings.
  • Maintain accurate documentation using Point Click Care (PCC) software.
  • Provide support and guidance to residents and families regarding care options.
  • Collaborate with interdisciplinary teams to address complex resident cases.
  • Monitor compliance with healthcare regulations and organizational standards.
  • Participate in staff training and development initiatives to enhance service quality.

Job Requirements

  • Master’s degree in Social Work (MSW) required; LMSW certification preferred.
  • Experience in geriatrics, long-term care, or healthcare preferred.
  • Proficiency in Point Click Care (PCC) software is advantageous.
  • Strong skills in case management and discharge planning.
  • Ability to assess resident needs and develop effective care plans.
  • Excellent communication and interpersonal skills for resident and family engagement.
