Overview
- Provide critical hospice care during after-hours shifts, addressing urgent physical and psycho-social patient needs effectively and compassionately.
- Coordinate with Call Center Triage and On Call Manager to ensure seamless care delivery and patient support.
- Respond promptly to triage visit requests, communicating effectively with the hospice care team and physicians.
- Implement and educate patients and caregivers on care plans, medication management, and symptom control strategies.
- Provide emotional support and guidance to families during patient deaths, ensuring compassionate care.
- Maintain compliance with New York State standards for professional registered nurses.
- Engage in both office and field settings, utilizing transportation services as needed for patient visits.
- Contribute to a collaborative and supportive hospice care environment, enhancing patient and family experiences.
Key Responsibilities & Duties
- Conduct scheduled and unscheduled patient visits to address urgent care needs during assigned shifts.
- Coordinate with triage and on-call teams to establish and implement safe care plans.
- Educate patients and caregivers on care plan adjustments, medication management, and safety protocols.
- Attend patient deaths to provide emotional support and guidance to families.
- Report patient visit outcomes and follow-up needs to the primary hospice team.
- Ensure compliance with professional nursing standards and hospice care guidelines.
- Utilize office and field resources effectively to deliver high-quality patient care.
- Collaborate with hospice physicians and care teams to address patient needs promptly.
Job Requirements
- Bachelor of Arts degree in Nursing from an accredited institution.
- Minimum of two years of experience in Medical-Surgical, Community Health, Hospice, or Oncology nursing preferred.
- Active New York State RN licensure and valid driver’s license preferred.
- Commitment to obtaining CHPN certification within one year of hire.
- Proficiency in Word, Excel, and Windows applications.
- Strong communication skills to coordinate with hospice care teams and physicians.
- Ability to provide compassionate care and emotional support to patients and families.
- Flexibility to work varied shifts, including weekends, as required.
