Job Description

Overview

  • Provide essential nursing care and support to patients in a long-term care setting, ensuring their safety and satisfaction.
  • Work under the supervision of a Registered Nurse or Clinical Care Manager, following established protocols and procedures.
  • Perform duties as a Licensed Practical Nurse (LPN), including medication administration and patient monitoring.
  • Participate in a rotating weekend schedule to ensure consistent patient care.
  • Complete a comprehensive four-day orientation program to familiarize with organizational practices.
  • Engage in in-person interviews to demonstrate qualifications and suitability for the role.
  • Commit to a consistent work schedule from 3:00 PM to 11:00 PM.
  • Ensure compliance with all regulatory and organizational standards for patient care.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed, ensuring accuracy and adherence to protocols.
  • Monitor patient conditions and report changes to the supervising nurse or physician.
  • Maintain accurate and detailed patient records, documenting care provided and observations.
  • Assist patients with daily living activities, promoting comfort and well-being.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Provide emotional support and education to patients and their families regarding care plans and procedures.
  • Ensure compliance with infection control and safety protocols within the care environment.
  • Participate in ongoing training and professional development opportunities to enhance skills.

Job Requirements

  • Valid NYS Licensed Practical Nurse (LPN) license required.
  • Minimum of two years of experience in long-term care or geriatric nursing.
  • Graduate of an approved practical nursing program with BCLS and IV certification.
  • High school diploma or GED equivalent required.
  • Commitment to working every other weekend as part of the schedule.
  • Strong communication and interpersonal skills to interact effectively with patients and staff.
  • Ability to work on-site during designated hours of 3:00 PM to 11:00 PM.
  • Completion of a four-day orientation program upon hiring.
