Job Description

Overview

  • Provide comprehensive nursing care to clients in home settings, ensuring compliance with state regulations and organizational policies.
  • Coordinate resources and services for assigned caseloads based on client needs and health conditions.
  • Conduct initial and ongoing client assessments to develop and implement effective care plans.
  • Collaborate with families, physicians, and care providers to optimize patient outcomes and satisfaction.
  • Document patient care activities accurately and in compliance with regulatory requirements.
  • Provide education, counseling, and training to patients and caregivers using evidence-based practices.
  • Supervise Home Health Aides, assigning duties based on client needs and care plans.
  • Participate in quality improvement initiatives to enhance patient care and organizational goals.
  • Travel to client homes across designated neighborhoods to deliver nursing care.

Key Responsibilities & Duties

  • Perform comprehensive assessments to determine client needs and develop individualized care plans.
  • Implement and evaluate care plans, ensuring adherence to medical and regulatory standards.
  • Coordinate with healthcare teams and external partners to provide seamless care.
  • Deliver education and training to clients and caregivers, promoting autonomy and dignity.
  • Supervise and train Home Health Aides, ensuring quality and compliance in care delivery.
  • Maintain accurate and timely documentation of patient care activities and outcomes.
  • Respond to emergencies and acute changes in patient conditions promptly and effectively.
  • Participate in organizational initiatives to improve patient satisfaction and care quality.
  • Conduct in-service training sessions for staff as required.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and current New York State RN license.
  • Minimum of two years of experience in home health care preferred.
  • Basic Cardiac Life Support certification required.
  • Strong organizational skills and ability to work independently and collaboratively.
  • Excellent communication skills with a commitment to professionalism and confidentiality.
  • Willingness to travel across designated neighborhoods to provide care.
  • Demonstrated knowledge of healthcare practices and regulatory compliance.
