Nurse Practitioner (Np) – Bronx Field/Hybrid – Spanish

Overview

• Provide primary care services to patients in home settings, focusing on geriatric and diverse populations.
• Utilize bilingual Spanish skills to communicate effectively with patients and their families.
• Collaborate with a multidisciplinary team to create individualized treatment plans.
• Support telemedicine operations and perform remote follow-ups when necessary.
• Commit to value-based care to improve patient outcomes and satisfaction.
• Maintain accurate documentation using the practice management system and EMR.
• Participate in on-call coverage for clinical escalations during holidays and weekends.
• Ensure compliance with licensing requirements and maintain active certifications.

Key Responsibilities & Duties

• Provide high-quality medical care in partnership with the clinical team.
• Perform diagnostic and therapeutic procedures within the scope of practice.
• Collaborate with the Medical Director to support clinical personnel and operations.
• Develop and implement patient-centric care models to address individual needs.
• Ensure effective communication with patients and community stakeholders.
• Participate in continuing medical education to stay updated on best practices.
• Provide consultations and manage patient care plans effectively.
• Travel to patient locations and provide care in various living environments.

Job Requirements

• Master of Science in Nursing from an accredited institution.
• Active Nurse Practitioner license in the State of New York.
• Minimum of 1 year of experience, with 3 years preferred.
• DEA Certification and clean record required.
• Proficiency in Spanish language is mandatory.
• Excellent communication and interpersonal skills.
• Ability to travel and work in diverse patient environments.
• Commitment to ethical practices and teamwork.
