Job Description

Overview

  • Coordinate home care services and ensure safe transitions for patients returning to the community.
  • Provide emotional and social support to patients and their families during rehabilitation.
  • Educate patients and families about entitlement benefits and assist with applications.
  • Maintain thorough documentation of activities and outcomes for all cases.
  • Collaborate with the care team to ensure comprehensive patient support.
  • Support residents in maintaining benefits throughout their rehabilitation stay.
  • Work on-site during regular business hours, Monday through Friday, 9 AM to 5 PM.

Key Responsibilities & Duties

  • Assess patient needs and develop individualized care plans.
  • Coordinate with healthcare providers to arrange necessary services.
  • Document patient progress and communicate updates to the care team.
  • Provide guidance on entitlement benefits and application processes.
  • Offer emotional support to patients and families during transitions.
  • Ensure compliance with all regulatory and organizational policies.
  • Participate in team meetings to discuss patient care strategies.

Job Requirements

  • Master’s degree in Social Work (MSW) is required.
  • Minimum of 2 years of experience in geriatrics or discharge planning preferred.
  • Strong case management skills and ability to assess patient needs.
  • Excellent customer service skills to support patients and families.
  • Proficient in documentation and maintaining accurate records.
  • Knowledge of entitlement benefits and application processes.
  • Ability to work effectively within a multidisciplinary team.
