Overview
- Deliver exceptional occupational therapy services to patients in a community setting, ensuring personalized care and treatment plans.
- Collaborate with multidisciplinary teams to provide holistic and coordinated patient care.
- Perform evaluations and assessments to monitor progress and adapt therapeutic approaches.
- Supervise and mentor therapy assistants to uphold high-quality care standards.
- Document patient health status and therapy outcomes in compliance with guidelines.
- Participate in interdisciplinary team meetings to ensure comprehensive care coordination.
- Promote patient independence through education and tailored interventions.
- Adhere to state regulations and organizational policies while delivering therapy services.
Key Responsibilities & Duties
- Develop and implement individualized treatment plans based on thorough patient evaluations.
- Adjust therapy plans and goals in response to patient progress and feedback.
- Conduct OASIS assessments accurately and in compliance with established standards.
- Provide supervision and guidance to occupational therapy assistants and aides.
- Communicate effectively with patients, families, and healthcare team members.
- Monitor and report changes in patient conditions to appropriate personnel promptly.
- Participate in team conferences to ensure coordinated care for complex cases.
- Perform additional duties as assigned to support organizational goals and patient care.
Job Requirements
- Master’s or Doctorate degree in Occupational Therapy from an accredited institution.
- Current and valid license to practice Occupational Therapy in New York State.
- Minimum of one year of professional experience in occupational therapy preferred.
- Proficiency in developing and implementing individualized treatment plans.
- Strong communication skills for effective interaction with patients and healthcare teams.
- Ability to supervise and evaluate therapy assistants and aides.
- Knowledge of OASIS assessment guidelines and documentation standards.
