Job Description

Overview

  • Deliver exceptional occupational therapy services to patients in a community setting, ensuring personalized care and treatment plans.
  • Collaborate with multidisciplinary teams to provide holistic and coordinated patient care.
  • Perform evaluations and assessments to monitor progress and adapt therapeutic approaches.
  • Supervise and mentor therapy assistants to uphold high-quality care standards.
  • Document patient health status and therapy outcomes in compliance with guidelines.
  • Participate in interdisciplinary team meetings to ensure comprehensive care coordination.
  • Promote patient independence through education and tailored interventions.
  • Adhere to state regulations and organizational policies while delivering therapy services.

Key Responsibilities & Duties

  • Develop and implement individualized treatment plans based on thorough patient evaluations.
  • Adjust therapy plans and goals in response to patient progress and feedback.
  • Conduct OASIS assessments accurately and in compliance with established standards.
  • Provide supervision and guidance to occupational therapy assistants and aides.
  • Communicate effectively with patients, families, and healthcare team members.
  • Monitor and report changes in patient conditions to appropriate personnel promptly.
  • Participate in team conferences to ensure coordinated care for complex cases.
  • Perform additional duties as assigned to support organizational goals and patient care.

Job Requirements

  • Master’s or Doctorate degree in Occupational Therapy from an accredited institution.
  • Current and valid license to practice Occupational Therapy in New York State.
  • Minimum of one year of professional experience in occupational therapy preferred.
  • Proficiency in developing and implementing individualized treatment plans.
  • Strong communication skills for effective interaction with patients and healthcare teams.
  • Ability to supervise and evaluate therapy assistants and aides.
  • Knowledge of OASIS assessment guidelines and documentation standards.
