Nurse Practitioner

Overview

• Deliver high-quality patient care by managing acute and chronic conditions, emphasizing health promotion and disease prevention.
• Collaborate with multidisciplinary teams to create and implement effective treatment plans ensuring optimal healthcare delivery.
• Participate in community outreach and health education initiatives to promote wellness and engage with diverse populations.
• Utilize advanced electronic medical record systems for accurate documentation and efficient patient management.
• Contribute to quality improvement projects aimed at enhancing clinical outcomes and patient satisfaction.
• Provide culturally sensitive care to diverse populations, including immigrants, ensuring inclusivity and respect.
• Prescribe evidence-based treatments and medications within the scope of licensure and professional standards.
• Stay current with certifications and comply with healthcare regulations to maintain professional excellence.

Key Responsibilities & Duties

• Conduct comprehensive assessments and manage acute and chronic medical conditions for adult patients.
• Perform mental health screenings and prescribe medications for conditions such as anxiety and depression.
• Order and interpret diagnostic tests to inform evidence-based treatment decisions.
• Advise patients on preventive care measures and treatment plans to promote overall wellness.
• Document patient interactions and care plans accurately in the electronic medical record system.
• Collaborate with healthcare professionals to ensure coordinated and comprehensive care delivery.
• Participate in team meetings and contribute to pre-visit planning activities.
• Engage in initiatives aimed at improving healthcare quality and patient outcomes.

Job Requirements

• Bachelor of Science in Nursing (BSN) with advanced degree in Nursing preferred.
• Certification as a Family Nurse Practitioner or Adult Nurse Practitioner.
• Valid NYS license and DEA certification required.
• Updated certifications in BLS and infection control.
• Proficiency in Spanish or an Asian language is highly desirable.
• Experience with PrEP/PEP, STI treatment, and mental health medication prescription preferred.
• Familiarity with electronic medical record systems and ambulatory care settings.
• AAHIVM certification and/or LGBT health certification is a plus.
• Minimum two years of experience as a Nurse Practitioner required.
