Job Description

Overview

  • Lead and mentor a dedicated nursing team in delivering exceptional hospice and palliative care services.
  • Ensure adherence to program philosophy, goals, and policies while maintaining high-quality patient care standards.
  • Collaborate with interdisciplinary teams to optimize patient and family goals of care.
  • Oversee staff assignments and scheduling in compliance with NHPCO and organizational staffing ratios.
  • Monitor and evaluate nursing documentation to meet regulatory requirements and organizational policies.
  • Provide leadership and guidance to nursing staff, fostering professional growth and excellence in care delivery.
  • Participate in community presentations to support admissions and outreach initiatives.
  • Maintain availability beyond core hours to address incidents and ensure continuity of care.

Key Responsibilities & Duties

  • Support the Associate Director of Patient Services in managing day-to-day operations of hospice nursing services.
  • Mentor RN Case Managers in pain and symptom management and patient-centered care planning.
  • Schedule and coordinate staff assignments to ensure optimal service delivery.
  • Evaluate nursing documentation for compliance with state, federal, and organizational standards.
  • Conduct community presentations in collaboration with admissions and outreach teams.
  • Provide leadership and mentoring to nursing staff, fostering professional development and excellence.
  • Ensure availability to respond to incidents and maintain continuity of care beyond core hours.
  • Collaborate with interdisciplinary teams to enhance patient and family care experiences.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution required.
  • Minimum of five years of hospice nursing experience with supervisory or leadership experience preferred.
  • New York State RN licensure required; CHPN certification preferred or obtainable within one year.
  • Proficiency in Word, Outlook, and Windows applications required.
  • Strong ability to supervise and mentor hospice nursing staff effectively.
  • Experience in triage, after-hours, or on-call home care settings highly valued.
  • Availability to respond to incidents beyond core hours as needed.
  • Excellent interpersonal and communication skills to work within interdisciplinary teams.
