Overview
- Lead and mentor a dedicated nursing team in hospice care services, ensuring quality and compassionate patient support.
- Collaborate with interdisciplinary teams to uphold the philosophy and goals of hospice and palliative care programs.
- Manage staff assignments and schedules to maintain compliance with industry standards and staffing ratios.
- Evaluate and enhance nursing documentation for adherence to policies and regulatory requirements.
- Provide mentorship in pain and symptom management, focusing on patient and family care goals.
- Participate in community presentations to promote hospice services and outreach efforts.
- Ensure availability for incident follow-ups beyond core hours to maintain service continuity.
- Support the Associate Director in leadership and operational guidance for hospice nursing staff.
Key Responsibilities & Duties
- Supervise and mentor nursing staff to deliver exceptional hospice and palliative care services.
- Coordinate staff assignments and schedules in line with organizational policies and NHPCO standards.
- Monitor nursing documentation quality and compliance with state and federal regulations.
- Empower RN Case Managers to excel in pain and symptom management practices.
- Collaborate with outreach departments to deliver impactful community presentations.
- Provide leadership and guidance to ensure seamless operational support during incidents.
- Maintain effective communication within interdisciplinary teams to achieve care objectives.
- Support the Associate Director in strategic planning and implementation of care services.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution required.
- Minimum of five years of hospice nursing experience essential for the role.
- Supervisory or leadership experience preferred to manage nursing teams effectively.
- Active New York State RN licensure required for practice.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
- Proficiency in Word, Outlook, and Windows applications necessary for operational tasks.
- Strong ability to supervise and mentor hospice nursing staff effectively.
- Experience in triage, on-call, or hospital AOC settings advantageous.
