Job Description

Overview

  • Provide comprehensive nursing care within an Adult Day Health Care Program, focusing on adults with chronic medical conditions and vision impairments.
  • Collaborate with a multidisciplinary team to deliver high-quality healthcare services and education to registrants.
  • Conduct nursing assessments, develop treatment plans, and coordinate healthcare services for registrants.
  • Engage in individual and group health education, promoting wellness and safety among registrants.
  • Ensure compliance with safety protocols and infection control measures within the program environment.
  • Support registrants and their families through health counseling and advocacy.
  • Participate in quality assurance and performance improvement initiatives to enhance program outcomes.
  • Maintain accurate documentation and records in compliance with regulatory standards.
  • Provide leadership and guidance in the absence of the Program Director.

Key Responsibilities & Duties

  • Conduct nursing assessments and develop individualized treatment plans for registrants.
  • Monitor registrants' medications and ensure proper inventory management of medical supplies.
  • Handle medical emergencies and educate staff on appropriate procedures.
  • Maintain a safe and clean environment, ensuring optimal condition of medical equipment.
  • Provide health education and counseling to registrants and their families.
  • Act as a liaison between the program, Medical Director, and registrants' private physicians.
  • Participate in interdisciplinary team activities to foster collaboration and improve care delivery.
  • Supervise nursing students and volunteers, providing guidance and support.
  • Ensure confidentiality of registrants' health information and compliance with security policies.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Current Registered Nurse license from the State Board of Nursing.
  • Minimum of 3 years of recent experience in Med/Surg, Long Term Care, or Community Health.
  • Proficiency in MS Word and excellent documentation skills.
  • Preferred certification in UAS-NY and demonstrated proficiency.
  • Strong communication skills and ability to work with adult and elderly populations.
  • Physical ability to perform tasks such as lifting, transferring, and assisting registrants.
  • Commitment to maintaining confidentiality and adhering to ethical standards.
  • Ability to lead and supervise in a healthcare setting.
