Overview
- Provide comprehensive nursing care within an Adult Day Health Care Program, focusing on adults with chronic medical conditions and vision impairments.
- Collaborate with a multidisciplinary team to deliver high-quality healthcare services and education to registrants.
- Conduct nursing assessments, develop treatment plans, and coordinate healthcare services for registrants.
- Engage in individual and group health education, promoting wellness and safety among registrants.
- Ensure compliance with safety protocols and infection control measures within the program environment.
- Support registrants and their families through health counseling and advocacy.
- Participate in quality assurance and performance improvement initiatives to enhance program outcomes.
- Maintain accurate documentation and records in compliance with regulatory standards.
- Provide leadership and guidance in the absence of the Program Director.
Key Responsibilities & Duties
- Conduct nursing assessments and develop individualized treatment plans for registrants.
- Monitor registrants' medications and ensure proper inventory management of medical supplies.
- Handle medical emergencies and educate staff on appropriate procedures.
- Maintain a safe and clean environment, ensuring optimal condition of medical equipment.
- Provide health education and counseling to registrants and their families.
- Act as a liaison between the program, Medical Director, and registrants' private physicians.
- Participate in interdisciplinary team activities to foster collaboration and improve care delivery.
- Supervise nursing students and volunteers, providing guidance and support.
- Ensure confidentiality of registrants' health information and compliance with security policies.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Current Registered Nurse license from the State Board of Nursing.
- Minimum of 3 years of recent experience in Med/Surg, Long Term Care, or Community Health.
- Proficiency in MS Word and excellent documentation skills.
- Preferred certification in UAS-NY and demonstrated proficiency.
- Strong communication skills and ability to work with adult and elderly populations.
- Physical ability to perform tasks such as lifting, transferring, and assisting registrants.
- Commitment to maintaining confidentiality and adhering to ethical standards.
- Ability to lead and supervise in a healthcare setting.
