Overview
- Provide nursing care in an adult medical day care program, focusing on chronic conditions and vision impairments.
- Work within a multidisciplinary team to deliver high-quality health services and education.
- Conduct nursing assessments and develop individualized treatment plans for registrants.
- Coordinate healthcare services and act as a liaison with registrants' private physicians.
- Ensure compliance with safety and infection control standards within the facility.
- Provide health education and counseling to registrants and their families.
- Supervise nursing students and volunteers, fostering professional development.
- Maintain confidentiality and adhere to healthcare information security policies.
- Perform general assistance tasks, including registrant escorting and meal service.
Key Responsibilities & Duties
- Conduct comprehensive nursing assessments and document findings accurately.
- Develop, implement, and evaluate nursing treatment plans for registrants.
- Monitor registrants' medication intake and maintain inventory standards.
- Handle medical emergencies and educate staff on emergency procedures.
- Maintain a clean and safe environment, ensuring optimal medical equipment condition.
- Lead health education sessions and provide individual counseling.
- Participate in quality assurance and performance improvement activities.
- Assist registrants with transportation and personal care needs.
- Act as Program Director in their absence, ensuring program continuity.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Current NYS Registered Nurse license with 3-5 years of relevant experience.
- Proficiency in MS Word and preferred certification in UAS-NY.
- Experience in Med/Surg, Long Term Care, or Community Health settings.
- Strong communication, documentation, and assessment skills.
- Ability to manage physical demands such as lifting and transferring registrants.
- Commitment to maintaining confidentiality and adhering to security policies.
- Warm personality with a passion for working with adult and elderly populations.
- Flexibility to perform interdisciplinary team tasks and general assistance duties.
