Job Description

Overview

  • Provide clinical nutrition services to residents in long-term and sub-acute care units.
  • Conduct comprehensive nutrition assessments and develop individualized care plans.
  • Collaborate with interdisciplinary teams to support resident health goals.
  • Ensure compliance with regulatory documentation and standards.
  • Educate residents, patients, and families on nutrition and dietary practices.
  • Monitor and evaluate the effectiveness of nutrition interventions.
  • Participate in care plan meetings and meal rounds to ensure satisfaction.
  • Support quality and safety initiatives within the Food and Nutrition Department.

Key Responsibilities & Duties

  • Conduct nutrition assessments for residents in long-term and sub-acute care units.
  • Develop and implement individualized nutrition care plans based on assessments.
  • Document medical records accurately and timely, including MDS 3.0.
  • Provide nutrition education to residents, patients, and their families.
  • Participate in interdisciplinary care plan meetings to align care strategies.
  • Conduct meal rounds to ensure quality, safety, and resident satisfaction.
  • Collaborate with healthcare staff to support resident care goals.
  • Stay updated on regulatory requirements and best practices in nutrition care.

Job Requirements

  • Registered Dietitian credential (ADA certification required).
  • Minimum of two years of clinical experience in long-term care preferred.
  • Knowledge of MDS 3.0 and regulatory documentation standards.
  • Strong communication and interdisciplinary collaboration skills.
  • Ability to develop and monitor individualized nutrition care plans.
  • Proficiency in medical record documentation and compliance practices.
  • High School Diploma or GED required.
  • Experience in conducting education sessions for residents and families.
