Job Description
Overview
- Provide clinical nutrition services to residents in long-term and sub-acute care units.
- Conduct comprehensive nutrition assessments and develop individualized care plans.
- Collaborate with interdisciplinary teams to support resident health goals.
- Ensure compliance with regulatory documentation and standards.
- Educate residents, patients, and families on nutrition and dietary practices.
- Monitor and evaluate the effectiveness of nutrition interventions.
- Participate in care plan meetings and meal rounds to ensure satisfaction.
- Support quality and safety initiatives within the Food and Nutrition Department.
Key Responsibilities & Duties
- Conduct nutrition assessments for residents in long-term and sub-acute care units.
- Develop and implement individualized nutrition care plans based on assessments.
- Document medical records accurately and timely, including MDS 3.0.
- Provide nutrition education to residents, patients, and their families.
- Participate in interdisciplinary care plan meetings to align care strategies.
- Conduct meal rounds to ensure quality, safety, and resident satisfaction.
- Collaborate with healthcare staff to support resident care goals.
- Stay updated on regulatory requirements and best practices in nutrition care.
Job Requirements
- Registered Dietitian credential (ADA certification required).
- Minimum of two years of clinical experience in long-term care preferred.
- Knowledge of MDS 3.0 and regulatory documentation standards.
- Strong communication and interdisciplinary collaboration skills.
- Ability to develop and monitor individualized nutrition care plans.
- Proficiency in medical record documentation and compliance practices.
- High School Diploma or GED required.
- Experience in conducting education sessions for residents and families.
