Job Description
Overview
- Provide comprehensive nutrition assessment and clinical services to residents in long-term and sub-acute care units.
- Collaborate with interdisciplinary healthcare teams to support resident health outcomes and care goals.
- Develop and implement individualized nutrition care plans tailored to resident needs and preferences.
- Ensure compliance with regulatory documentation and standards, including MDS 3.0 requirements.
- Provide nutrition education to residents, patients, and their families as necessary.
- Participate in interdisciplinary care plan meetings to enhance resident care strategies.
- Conduct meal rounds to ensure quality, safety, and satisfaction of residents.
- Work on-site in a collaborative healthcare environment, offering full-time and part-time opportunities.
Key Responsibilities & Duties
- Perform nutrition assessments and provide clinical nutrition services for long-term and sub-acute care residents.
- Complete accurate and timely medical record documentation, including MDS 3.0 compliance.
- Develop, implement, and monitor individualized nutrition care plans for residents.
- Educate residents, patients, and family members on nutrition and dietary practices.
- Participate in interdisciplinary care plan meetings to coordinate resident care.
- Conduct meal rounds to ensure adherence to quality and safety standards.
- Collaborate with nursing, medical, and rehabilitation staff to achieve resident care goals.
- Maintain knowledge of regulatory requirements and ensure compliance in all activities.
Job Requirements
- Registered Dietitian credential from the American Dietetic Association (ADA) required.
- Minimum of two years of clinical experience in long-term care preferred.
- Knowledge of MDS 3.0 and regulatory requirements is highly desirable.
- Strong communication, documentation, and interdisciplinary collaboration skills.
- Ability to work on-site in a healthcare environment with occasional weekend shifts.
- High School Diploma or GED required; additional certifications preferred.
- Commitment to resident-focused care and quality improvement initiatives.
