Overview
- Provide in-home rehabilitation services to patients, focusing on improving functional outcomes and adhering to physician orders and regulatory standards.
- Conduct comprehensive patient evaluations, develop individualized treatment plans, and implement goal-oriented therapy programs.
- Collaborate with physicians and interdisciplinary team members to coordinate effective patient care and ensure optimal outcomes.
- Educate patients and caregivers on home exercise programs and care plans to promote recovery and independence.
- Maintain accurate and timely documentation, including evaluations, progress notes, reassessments, and discharge summaries.
- Monitor patient progress, adjust care plans as needed, and ensure services align with prescribed frequency and duration.
- Supervise Physical Therapist Assistants (PTAs) and ensure compliance with oversight and documentation requirements.
- Participate in discharge planning and provide recommendations for continued care needs.
Key Responsibilities & Duties

- Deliver skilled physical therapy services according to physician plan of care and organizational policies.
- Maintain accurate and timely documentation, including evaluations, progress notes, and discharge summaries.
- Monitor patient progress and adjust care plans to support goal achievement and compliance.
- Collaborate with physicians and interdisciplinary team members to coordinate patient care effectively.

Job Requirements
- Bachelor’s Degree in Physical Therapy required; Doctorate in Physical Therapy (DPT) preferred.
- Minimum of 6 months of Physical Therapy experience; 3 years preferred.
- Active Physical Therapist License in New Jersey and AHA Basic Life Support (BLS) Certification.
- Basic computer proficiency and experience with electronic medical records (EMR).
- Ability to lift, push, and pull up to 50 lbs and perform physical work for up to 8 hours per day.
- Strong organizational skills to manage scheduling requirements and maintain productivity standards.
- Commitment to providing high-quality patient care and achieving functional outcomes.
- Effective communication skills to collaborate with interdisciplinary teams and educate patients and caregivers.
