Overview
- Provide in-home occupational therapy services to patients, focusing on individualized, goal-oriented treatment plans.
- Ensure safe, effective, and patient-centered care in compliance with physician orders and regulatory standards.
- Collaborate with interdisciplinary teams to optimize patient outcomes and coordinate care.
- Utilize electronic medical records (EMR) for accurate and timely documentation of patient progress.
- Educate patients and caregivers on functional independence strategies and home programs.
- Participate in discharge planning to ensure continuity of care and recovery support.
- Maintain compliance with organizational policies and procedures to uphold care standards.
Key Responsibilities & Duties
- Conduct comprehensive patient evaluations in home settings to assess rehabilitation needs.
- Develop and implement treatment plans with measurable short- and long-term goals.
- Provide skilled occupational therapy services according to physician-prescribed care plans.
- Monitor patient progress and adjust treatment plans based on clinical indications.
- Document evaluations, progress notes, and discharge summaries accurately in EMR systems.
- Collaborate with physicians and care teams to ensure coordinated care and referrals.
- Educate patients and caregivers on strategies to enhance functional independence.
- Participate in discharge planning to ensure effective transition and continuity of care.
Job Requirements
- Bachelor’s Degree in Occupational Therapy from an accredited institution.
- Minimum of 1 year of experience in occupational therapy; 3 years preferred.
- Active Occupational Therapist License in New Jersey.
- AHA Basic Life Support (BLS) Certification is required.
- Proficiency in using electronic medical records (EMR) systems.
- Ability to lift, push, and pull up to 50 lbs and perform clinical duties for extended periods.
- Preferred qualifications include a Master’s Degree in Occupational Therapy and home care experience.
