Job Description

Overview

  • Provide in-home occupational therapy services to patients, focusing on individualized, goal-oriented treatment plans.
  • Ensure safe, effective, and patient-centered care in compliance with physician orders and regulatory standards.
  • Collaborate with interdisciplinary teams to optimize patient outcomes and coordinate care.
  • Utilize electronic medical records (EMR) for accurate and timely documentation of patient progress.
  • Educate patients and caregivers on functional independence strategies and home programs.
  • Participate in discharge planning to ensure continuity of care and recovery support.
  • Maintain compliance with organizational policies and procedures to uphold care standards.

Key Responsibilities & Duties

  • Conduct comprehensive patient evaluations in home settings to assess rehabilitation needs.
  • Develop and implement treatment plans with measurable short- and long-term goals.
  • Provide skilled occupational therapy services according to physician-prescribed care plans.
  • Monitor patient progress and adjust treatment plans based on clinical indications.
  • Document evaluations, progress notes, and discharge summaries accurately in EMR systems.
  • Collaborate with physicians and care teams to ensure coordinated care and referrals.
  • Educate patients and caregivers on strategies to enhance functional independence.
  • Participate in discharge planning to ensure effective transition and continuity of care.

Job Requirements

  • Bachelor’s Degree in Occupational Therapy from an accredited institution.
  • Minimum of 1 year of experience in occupational therapy; 3 years preferred.
  • Active Occupational Therapist License in New Jersey.
  • AHA Basic Life Support (BLS) Certification is required.
  • Proficiency in using electronic medical records (EMR) systems.
  • Ability to lift, push, and pull up to 50 lbs and perform clinical duties for extended periods.
  • Preferred qualifications include a Master’s Degree in Occupational Therapy and home care experience.
