Overview
- Provide integrated behavioral health care using brief, solution-focused interventions for medical and dental patients.
- Collaborate with interdisciplinary care teams to address psychosocial stressors and improve patient adherence to treatment plans.
- Enhance patients' wellbeing by addressing depression, anxiety, and other mental health concerns.
- Refer patients to external providers for traditional counseling or ongoing therapy as needed.
- Contribute to an innovative healthcare model serving diverse and underserved populations.
- Work in a federally qualified health center providing care regardless of insurance or ability to pay.
- Utilize evidence-based behavioral health assessment and treatment modalities in a primary care setting.
Key Responsibilities & Duties
- Collaborate with primary care teams to provide comprehensive behavioral health services to patients.
- Perform patient evaluations, determine differential diagnoses, and plan interventions.
- Document care provided and maintain confidentiality in compliance with organizational standards.
- Facilitate ethical decision-making processes and consult with care teams as needed.
- Conduct comprehensive assessments and refer patients for further care within the organization or community.
- Utilize teaching-learning theory to educate patients and families, influencing outcomes positively.
- Identify system-specific issues requiring evaluation and contribute to performance improvement processes.
- Report to the Director of Integrated Behavioral Health and adhere to accreditation guidelines.
Job Requirements
- Master of Social Work degree with clinical licensure as LCSW or LPC-MHSP.
- Minimum of 6 months of experience required; 2+ years preferred.
- Proficiency in Spanish, Arabic, or Kurdish is preferred.
- Certification or experience in trauma, co-occurring disorders, and evidence-based therapies preferred.
- Ability to work independently and communicate effectively with patients and care teams.
- Experience in integrated healthcare settings and innovative delivery methods preferred.
- Flexibility and motivation to work in a primary care environment.
