Overview
- Provide compassionate care to hospice patients, ensuring their physical and emotional needs are met effectively.
- Develop and implement personalized nursing care plans for end-of-life patients.
- Collaborate with interdisciplinary teams to deliver holistic patient care.
- Supervise and train home health aides and personal care workers in patient homes.
- Conduct thorough physical and psychosocial assessments to determine patient needs.
- Request and manage physician orders for adjustments in care plans as required.
- Ensure clinical data is accurately inputted and maintained in patient records.
- Provide education and support to patients and families during challenging times.
Key Responsibilities & Duties
- Perform comprehensive assessments of hospice patients' physical and emotional conditions.
- Develop and implement individualized nursing care plans tailored to patient needs.
- Supervise and evaluate the performance of home health aides and personal care workers.
- Collaborate with physicians to adjust care plans and ensure optimal patient outcomes.
- Provide hands-on care and support to patients in their homes.
- Maintain accurate and timely clinical documentation in patient records.
- Educate patients and families about end-of-life care and available resources.
- Participate in interdisciplinary team meetings to coordinate patient care effectively.
Job Requirements
- Associate of Science in Nursing (AS) or equivalent educational background.
- Minimum of 2 years of clinical experience, preferably in hospice or acute care settings.
- Proficiency in clinical data entry and record management systems.
- Strong communication, teaching, and listening skills to support patients and families.
- Ability to work collaboratively within an interdisciplinary team environment.
- Experience in supervising and training home health aides and personal care workers.
- Hospice experience preferred; home care and acute care experience considered.
