Job Description

Overview

  • Provide compassionate care to hospice patients, ensuring their physical and emotional needs are met effectively.
  • Develop and implement personalized nursing care plans for end-of-life patients.
  • Collaborate with interdisciplinary teams to deliver holistic patient care.
  • Supervise and train home health aides and personal care workers in patient homes.
  • Conduct thorough physical and psychosocial assessments to determine patient needs.
  • Request and manage physician orders for adjustments in care plans as required.
  • Ensure clinical data is accurately inputted and maintained in patient records.
  • Provide education and support to patients and families during challenging times.

Key Responsibilities & Duties

  • Perform comprehensive assessments of hospice patients' physical and emotional conditions.
  • Develop and implement individualized nursing care plans tailored to patient needs.
  • Supervise and evaluate the performance of home health aides and personal care workers.
  • Collaborate with physicians to adjust care plans and ensure optimal patient outcomes.
  • Provide hands-on care and support to patients in their homes.
  • Maintain accurate and timely clinical documentation in patient records.
  • Educate patients and families about end-of-life care and available resources.
  • Participate in interdisciplinary team meetings to coordinate patient care effectively.

Job Requirements

  • Associate of Science in Nursing (AS) or equivalent educational background.
  • Minimum of 2 years of clinical experience, preferably in hospice or acute care settings.
  • Proficiency in clinical data entry and record management systems.
  • Strong communication, teaching, and listening skills to support patients and families.
  • Ability to work collaboratively within an interdisciplinary team environment.
  • Experience in supervising and training home health aides and personal care workers.
  • Hospice experience preferred; home care and acute care experience considered.
  • Commitment to providing compassionate care to patients at the end stage of life.
