Overview
- Deliver exceptional nursing care in a supportive adult day care environment, focusing on registrants' health and wellness.
- Collaborate with a multidisciplinary team to create personalized care plans for registrants.
- Conduct comprehensive nursing assessments and monitor registrants' health status effectively.
- Provide health education and counseling to registrants, promoting informed health decisions.
- Ensure proper medication management and compliance with inventory standards.
- Act as a liaison between registrants, their physicians, and the program's Medical Director.
- Participate in quality assurance activities to enhance care delivery standards.
- Support registrants with personal care and mobility assistance as required.
Key Responsibilities & Duties
- Perform nursing assessments and develop, implement, and evaluate individualized treatment plans.
- Monitor registrants' medication schedules and maintain inventory management protocols.
- Respond promptly to medical emergencies and educate staff on safety procedures.
- Provide health counseling and lead group education sessions tailored to registrants' needs.
- Supervise nursing students and volunteers, ensuring effective training and mentorship.
- Collaborate with interdisciplinary teams to optimize registrants' care plans.
- Participate in staff meetings and contribute to program development initiatives.
- Assist registrants with mobility, meals, and transportation arrangements as needed.
Job Requirements
- Associate of Applied Science (AAS) degree from an accredited nursing institution.
- Current NYS Registered Nurse license; UAS-NY certification preferred.
- 3-5 years of experience in Med/Surg, Long Term Care, or Community Health settings.
- Proficiency in MS Word and excellent documentation skills are required.
- Strong communication skills and ability to engage with elderly populations effectively.
- Physical capability to assist registrants with mobility and equipment handling.
- Commitment to confidentiality and adherence to health information security policies.
- Experience in interdisciplinary collaboration and program development preferred.
