Overview

  • Provide compassionate social work services to hospice patients and families, addressing psychosocial needs and enhancing their quality of life.
  • Collaborate with interdisciplinary teams to develop personalized care plans tailored to diverse patient needs.
  • Support patients and families through pre- and post-bereavement phases with empathy and professionalism.
  • Act as a liaison between hospice staff and external care facilities to ensure seamless transitions of care.
  • Engage in field-based services across Manhattan, providing vital support in various settings.

Key Responsibilities & Duties

  • Develop and implement comprehensive social work care plans for hospice patients in various care settings.
  • Provide bereavement counseling and emotional support to families during hospice care.
  • Coordinate with inpatient and nursing home staff to ensure continuity and quality of care.
  • Maintain accurate clinical records and utilize electronic systems for efficient data management.
  • Offer supervision hours to candidates pursuing LCSW licensure, fostering professional growth.
  • Participate in interdisciplinary meetings to review patient progress and refine care strategies.
  • Ensure compliance with healthcare regulations and state licensure requirements.
  • Contribute to the continuous improvement of hospice care practices and patient outcomes.

Job Requirements

  • Master of Social Work degree from an accredited institution is required.
  • New York State LMSW or LCSW licensure is mandatory.
  • Minimum of one year of experience in healthcare social work is essential.
  • Proficiency in Microsoft Word and Excel is preferred for documentation tasks.
  • Strong organizational, communication, and interpersonal skills are vital for success.
  • Experience in hospice or palliative care is highly desirable.
  • Ability to work effectively within a collaborative interdisciplinary team environment.
  • Fieldwork in Manhattan is required, with occasional weekend coverage.
