Field Nurse (Rn)

Overview

• Provide skilled nursing care and clinical oversight to home care clients in Northern New Jersey.
• Conduct patient assessments, medication management, and develop individualized care plans.
• Supervise Certified Home Health Aides and coordinate care with interdisciplinary teams.

Key Responsibilities & Duties

• Conduct start of care visits and comprehensive nursing assessments for new patients.
• Perform medication management, patient education, and physical assessments.
• Develop and implement individualized Plans of Care (POC) tailored to patient needs.
• Supervise and provide guidance to Certified Home Health Aides (CHHAs).
• Coordinate care with physicians, families, and interdisciplinary teams for optimal outcomes.
• Maintain accurate and timely clinical documentation for all patient interactions.
• Monitor patient progress and revise care plans as needed to ensure quality care.
• Participate in staff meetings, case conferences, and quality improvement initiatives.

Job Requirements

• Active Registered Nurse (RN) license in New Jersey in good standing.
• Bachelor of Science in Nursing (BSN) from an accredited program.
• Minimum of 1 year of RN experience, preferably in home care settings.
• Current CPR certification and strong clinical assessment skills.
• Ability to travel throughout Bergen, Passaic, Essex, Union, and Morris Counties.
• Reliable transportation and excellent communication skills.
• Proficiency in maintaining accurate and timely clinical documentation.
