Overview
- Provide skilled nursing care and clinical oversight to home care clients in Northern New Jersey.
- Conduct patient assessments, medication management, and develop individualized care plans.
- Supervise Certified Home Health Aides and coordinate care with interdisciplinary teams.

- Participate in staff meetings, case conferences, and quality improvement initiatives.
- Ensure compliance with agency, state, and federal home care regulations.
Key Responsibilities & Duties
- Conduct start of care visits and comprehensive nursing assessments for new patients.
- Perform medication management, patient education, and physical assessments.
- Develop and implement individualized Plans of Care (POC) tailored to patient needs.
- Supervise and provide guidance to Certified Home Health Aides (CHHAs).
- Coordinate care with physicians, families, and interdisciplinary teams for optimal outcomes.
- Maintain accurate and timely clinical documentation for all patient interactions.
- Monitor patient progress and revise care plans as needed to ensure quality care.
- Participate in staff meetings, case conferences, and quality improvement initiatives.
Job Requirements
- Active Registered Nurse (RN) license in New Jersey in good standing.
- Bachelor of Science in Nursing (BSN) from an accredited program.
- Minimum of 1 year of RN experience, preferably in home care settings.
- Current CPR certification and strong clinical assessment skills.
- Ability to travel throughout Bergen, Passaic, Essex, Union, and Morris Counties.
- Reliable transportation and excellent communication skills.
- Proficiency in maintaining accurate and timely clinical documentation.
