Overview
- Coordinate and deliver high-quality services to program patients, ensuring their health education and care plans are effectively managed.

- Provide individual health education sessions and psychoeducational support groups to enhance patient understanding and engagement.
- Collaborate with program managers and staff to ensure seamless service delivery and adherence to organizational standards.
- Monitor patient satisfaction through surveys and reports, using feedback to improve program benchmarks.
- Maintain accurate records of patient interactions, attendance, and program activities in designated databases.
- Participate in quality assurance initiatives and continuous quality improvement tasks to enhance service delivery.
- Represent the organization at various venues to promote services and build community relationships.
Key Responsibilities & Duties

Job Requirements
- Bachelor’s degree in a related field with two years of relevant experience required.
- Experience working with target populations such as individuals with HIV/AIDS, mental illness, or substance use disorders preferred.
- Strong organizational, written, and verbal communication skills essential for effective service delivery.
- Proficiency in computer literacy and ability to adapt to reporting platforms within deadlines.
- Ability to work collaboratively and independently in a fast-paced environment.
