Job Description

Overview

  • Provide comprehensive primary and preventive healthcare services to patients in an outpatient neuro practice setting.
  • Conduct advanced health assessments, prescribe treatments, and establish care plans for diverse patient populations.
  • Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
  • Promote health literacy and culturally competent care through patient education and counseling.
  • Participate in quality improvement initiatives and contribute to clinical meetings and case conferences.
  • Maintain accurate electronic medical records and ensure compliance with regulatory standards.
  • Support the mission and values of the healthcare organization by delivering patient-centered care.
  • Engage in professional development activities to maintain licensure and certifications.

Key Responsibilities & Duties

  • Conduct comprehensive patient assessments, including medical histories, physical exams, and diagnostic evaluations.
  • Diagnose acute and chronic medical conditions and develop evidence-based treatment plans.
  • Prescribe medications and therapeutic interventions in compliance with state and federal regulations.
  • Manage chronic disease conditions and provide preventive care, immunizations, and screenings.
  • Educate patients and families on wellness, treatment adherence, and lifestyle modifications.
  • Coordinate referrals and collaborate with specialists and social services for comprehensive care.
  • Participate in community outreach, health fairs, and patient education events.
  • Ensure compliance with HIPAA, OSHA, and infection control standards while maintaining ethical practice.

Job Requirements

  • Current New York State Nurse Practitioner license and registration.
  • National NP certification in an appropriate specialty, such as Family or Adult Health.
  • Master’s or Doctoral Degree in Nursing from an accredited program.
  • Current DEA registration or ability to obtain within 30 days of hire.
  • Minimum one year of clinical experience preferred, with proficiency in EMR systems.
  • Strong clinical judgment, communication, and organizational skills.
  • Commitment to health equity, patient advocacy, and culturally competent care.
  • Ability to work effectively in a fast-paced clinical environment.
