Overview
- Lead and mentor nursing staff to ensure high-quality hospice and palliative care services aligned with program philosophy and goals.
- Manage staff schedules and assignments in compliance with industry standards and organizational staffing ratios.
- Provide guidance in pain and symptom management, focusing on patient and family goals of care.
- Monitor and evaluate nursing documentation to ensure compliance with policies and regulations.
- Collaborate with interdisciplinary teams to achieve optimal patient outcomes and care coordination.
- Participate in community outreach and presentations to promote hospice services.
- Maintain availability beyond core hours to address incidents and ensure continuity of care.
- Support the Associate Director in operational and strategic initiatives within the hospice program.
Key Responsibilities & Duties
- Provide leadership and mentoring to nursing staff, ensuring adherence to hospice care standards and policies.
- Develop and manage staff schedules, ensuring adequate coverage and compliance with staffing ratios.
- Evaluate nursing documentation for accuracy and compliance with regulatory requirements.
- Empower RN Case Managers in delivering patient-centered care and achieving family goals.
- Collaborate with admissions and outreach departments for community presentations and engagement.
- Ensure readiness and availability to address incidents and follow up as needed beyond regular hours.
- Support operational initiatives and assist the Associate Director in achieving program objectives.
- Maintain effective communication within interdisciplinary teams to enhance patient care coordination.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution required.
- Minimum of five years of hospice nursing experience; supervisory experience preferred.
- Active New York State RN licensure required; CHPN certification preferred.
- Proficiency in Word, Outlook, and Windows; ability to supervise and mentor staff effectively.
- Experience in triage, on-call, or after-hours care within hospice or hospital settings.
- Strong leadership skills and ability to work within interdisciplinary teams to achieve care goals.
- Commitment to maintaining availability for incident follow-ups beyond core hours.
- Demonstrated ability to manage schedules and ensure compliance with staffing ratios.
