Job Description

Overview

  • Provide comprehensive healthcare services to residents, conducting assessments and managing chronic and acute conditions.
  • Collaborate with interdisciplinary teams to develop individualized care plans tailored to residents' needs.
  • Prescribe medications and treatments in compliance with state regulations and facility policies.
  • Educate residents and families on health maintenance, disease prevention, and self-care strategies.
  • Participate in quality improvement initiatives to enhance care outcomes and resident satisfaction.
  • Maintain accurate documentation of assessments, interventions, and communications in electronic medical records.
  • Order and interpret diagnostic tests to support diagnosis and treatment planning.
  • Engage in care initiation for chronic care management services to optimize resident health.

Key Responsibilities & Duties

  • Conduct physical examinations, medical histories, and medication reconciliation for residents.
  • Develop and implement personalized care plans in collaboration with healthcare teams.
  • Prescribe medications and treatments within scope of practice and regulatory guidelines.
  • Provide timely interventions for chronic conditions and acute illnesses.
  • Educate residents and families on health-related topics to promote wellness.
  • Participate in interdisciplinary meetings to review and enhance care strategies.
  • Order and interpret diagnostic tests to inform treatment decisions.
  • Maintain compliance with documentation standards in electronic health records.

Job Requirements

  • Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
  • Unrestricted New York State Nurse Practitioner license and DEA permit.
  • Eligibility to provide reimbursable services under Medicare and Medicaid.
  • Minimum of 2 years of clinical experience; 3 years preferred.
  • Experience with eClinicalWorks (eCW) electronic health records is advantageous.
  • Strong knowledge of chronic care management and acute illness interventions.
  • Excellent communication skills for resident and family education.
  • Commitment to quality improvement and interdisciplinary collaboration.
