Overview
- Provide comprehensive healthcare services to residents, conducting assessments and managing chronic and acute conditions.
- Collaborate with interdisciplinary teams to develop individualized care plans tailored to residents' needs.
- Prescribe medications and treatments in compliance with state regulations and facility policies.
- Educate residents and families on health maintenance, disease prevention, and self-care strategies.
- Participate in quality improvement initiatives to enhance care outcomes and resident satisfaction.
- Maintain accurate documentation of assessments, interventions, and communications in electronic medical records.
- Order and interpret diagnostic tests to support diagnosis and treatment planning.
- Engage in care initiation for chronic care management services to optimize resident health.
Key Responsibilities & Duties
- Conduct physical examinations, medical histories, and medication reconciliation for residents.
- Develop and implement personalized care plans in collaboration with healthcare teams.
- Prescribe medications and treatments within scope of practice and regulatory guidelines.
- Provide timely interventions for chronic conditions and acute illnesses.
- Educate residents and families on health-related topics to promote wellness.
- Participate in interdisciplinary meetings to review and enhance care strategies.
- Order and interpret diagnostic tests to inform treatment decisions.
- Maintain compliance with documentation standards in electronic health records.
Job Requirements
- Master’s degree in Nursing and completion of an accredited Nurse Practitioner program.
- Unrestricted New York State Nurse Practitioner license and DEA permit.
- Eligibility to provide reimbursable services under Medicare and Medicaid.
- Minimum of 2 years of clinical experience; 3 years preferred.
- Experience with eClinicalWorks (eCW) electronic health records is advantageous.
- Strong knowledge of chronic care management and acute illness interventions.
- Excellent communication skills for resident and family education.
- Commitment to quality improvement and interdisciplinary collaboration.
