- Participate in case conferences to ensure effective case management and discharge planning.

- Demonstrate knowledge of reimbursement practices for Medicare, Medicaid, and other payors.
- Maintain patient confidentiality and adhere to state and federal regulations.
- Participate in quality improvement activities and staff orientation processes.
- Serve as an on-call supervisor, ensuring continuity of care during off-hours.
Key Responsibilities & Duties
- Coordinate and assess home healthcare needs for referred patients, ensuring timely intake processes.
- Collaborate with physicians and nurses to establish appropriate levels of care and treatment plans.
- Order necessary medical equipment, supplies, and ancillary services for patient care.
- Schedule and assign visits for field staff, ensuring effective case management.
- Conduct case conferences to ensure coordination and timely discharge planning.
- Review admission and recertification documentation to ensure compliance and appropriateness.
- Provide clear direction to support staff, ensuring continuity of care.
- Demonstrate cultural sensitivity and maintain courteous interactions with all stakeholders.
- Participate in performance improvement activities and maintain knowledge of clinical documentation standards.
Job Requirements
- License to practice as a registered nurse; BSN preferred.
- Minimum of three years of nursing experience; two years in home health preferred.
- Knowledge of OASIS and clinical documentation standards.
- Effective verbal and written communication skills.
- Excellent problem-solving and organizational skills.
- Ability to manage multiple responsibilities and set priorities effectively.
- Commitment to customer relations and quality service.
- Demonstrated knowledge of state and federal regulations.
