Overview
- Provide comprehensive nursing care to individuals, ensuring their health needs are met and monitored effectively.
- Coordinate medical care, including follow-up appointments and assessments, to maintain optimal health outcomes.
- Develop and update individual-specific nursing plans and protocols based on health status changes.
- Collaborate with interdisciplinary teams, families, and clinicians to ensure holistic care delivery.
- Supervise medication administration and provide training to staff for compliance with regulations.
- Monitor dietary and nutritional needs, offering guidance to ensure proper meal management.
- Participate in clinical reviews and evaluations to enhance care quality and outcomes.

Key Responsibilities & Duties
- Assess and monitor individuals' health status, documenting findings and arranging follow-ups as needed.
- Communicate effectively with physicians, families, and team members regarding health concerns and updates.
- Accompany individuals to medical appointments, advocating for their health needs.
- Develop and revise nursing plans and protocols to reflect current health conditions.
- Supervise medication administration and ensure compliance with federal and state regulations.
- Monitor and manage medication storage and disposal procedures to maintain safety standards.
- Provide training and certification for staff in medication administration processes.
- Participate in discharge planning meetings, offering recommendations for follow-up care.
Job Requirements
- Bachelor of Science in Nursing (BSN) and current RN licensure in New York State.
- Minimum of 2 years of nursing experience, preferably with developmental disabilities.
- CPR certification and valid NY State or Tri-State Driver's License required.
- Strong written and oral communication skills to effectively collaborate with teams.
- Ability to prioritize tasks and work efficiently in a team-oriented environment.
- Knowledge of OSHA guidelines and infection control practices.
- Experience in developing and implementing nursing plans and protocols.
