Overview
- Provide comprehensive healthcare services to individuals experiencing homelessness in a community health setting.
- Collaborate with multidisciplinary teams to ensure holistic patient care and continuity of services.

- Develop and implement individualized care plans, including preventive and therapeutic measures.

Key Responsibilities & Duties
- Conduct physical assessments, diagnose medical conditions, and prescribe treatments within scope of practice.
- Administer medications and nursing treatments as required, ensuring patient safety.
- Coordinate referrals to specialty and consultative medical services when necessary.
- Collaborate with supervising physicians to ensure adherence to protocols and standards.
- Document patient encounters and follow-up actions in compliance with agency policies.
- Participate in after-hours telephone triage and emergency response activities.
- Supervise and mentor healthcare staff to enhance service delivery and patient outcomes.
- Engage in infection control practices and maintain compliance with OSHA standards.
Job Requirements
- Master of Science in Nursing and Family Nurse Practitioner certification required.
- Current New York State License and DEA registration mandatory.
- Minimum of 1-2 years of experience in ambulatory care or community health settings.
- Strong knowledge of family medicine practices and chronic disease management.
- Proficiency in patient care documentation and regulatory compliance.
- Ability to work collaboratively in a multidisciplinary team environment.
