Homecare Nurse

Key Responsibilities & Duties

• Conduct patient assessments and develop individualized care plans based on physician orders.
• Administer medications and perform clinical interventions in line with care guidelines.
• Document patient care activities accurately and promptly in compliance with policies.
• Educate patients and caregivers to support care plan goals and health management.
• Coordinate with interdisciplinary teams to ensure continuity and quality of care.
• Supervise home health aides and conduct required evaluations and visits.
• Monitor patient progress and escalate changes in condition to clinical leadership.
• Adhere to regulatory guidelines and professional nursing standards.

Job Requirements

• Active New Jersey Registered Nurse (RN) license required.
• Bachelor of Science in Nursing (BSN) preferred; ADN or Nursing Diploma acceptable.
• Minimum 6 months RN experience or 1 year LPN experience in home care or hospice preferred.
• Basic Life Support (BLS) certification through AHA required.
• Proficiency in computer systems and documentation software.
• Ability to lift/push/pull up to 50 lbs and work on-site.
• Strong communication, critical thinking, and time management skills.
