Key Responsibilities & Duties

• Deliver skilled nursing care in patients' homes, including assessments and care planning.
• Maintain accurate documentation of patient status, interventions, and progress.
• Perform medication reconciliation and update care plans as needed.
• Coordinate care with physicians and interdisciplinary team members.
• Educate patients and caregivers based on individualized treatment goals.
• Monitor compliance with payer requirements and authorization guidelines.
• Supervise and evaluate home health aide staff, ensuring quality care delivery.
• Communicate changes in patient condition to clinical leadership promptly.

Job Requirements

• Active New Jersey Registered Nurse (RN) license required.
• Bachelor of Science in Nursing (BSN) preferred; ADN or Nursing Diploma acceptable.
• Minimum of 6 months RN experience or 1 year LPN experience.
• Basic Life Support (BLS) certification through AHA required.
• Proficiency in electronic documentation and basic computer skills.
• Experience in home care, hospice, med-surg, or wound care preferred.
• Strong communication and organizational skills to coordinate patient care.
