Key Responsibilities & Duties
- Deliver skilled nursing care in patients' homes, including assessments and care planning.
- Maintain accurate documentation of patient status, interventions, and progress.
- Perform medication reconciliation and update care plans as needed.
- Coordinate care with physicians and interdisciplinary team members.
- Educate patients and caregivers based on individualized treatment goals.
- Monitor compliance with payer requirements and authorization guidelines.
- Supervise and evaluate home health aide staff, ensuring quality care delivery.
- Communicate changes in patient condition to clinical leadership promptly.
Job Requirements
- Active New Jersey Registered Nurse (RN) license required.
- Bachelor of Science in Nursing (BSN) preferred; ADN or Nursing Diploma acceptable.
- Minimum of 6 months RN experience or 1 year LPN experience.
- Basic Life Support (BLS) certification through AHA required.
- Proficiency in electronic documentation and basic computer skills.
- Experience in home care, hospice, med-surg, or wound care preferred.
- Strong communication and organizational skills to coordinate patient care.
