Job Description

Overview

  • Lead quality management initiatives for hospice and palliative care services, ensuring compliance with regulatory standards and enhancing patient care quality.
  • Develop and implement Quality Assurance/Performance Improvement (QAPI) plans aligned with industry standards and patient needs.
  • Collaborate with senior leadership to recommend strategic quality initiatives and report on key quality indicators.
  • Utilize data analysis tools to monitor clinical performance and identify improvement opportunities.
  • Ensure compliance with Federal and NYS regulations, serving as a resource on regulatory matters.
  • Oversee CMS Hospice Quality Reporting Program, implementing performance improvement projects based on data insights.
  • Manage staffing and resources to support departmental services, including recruitment and performance management.
  • Facilitate collaboration across clinical and operational departments to enhance care processes and outcomes.

Key Responsibilities & Duties

  • Develop and maintain quality management programs for hospice services, ensuring alignment with patient needs and regulatory requirements.
  • Collaborate with leadership to establish clinical performance indicators and manage benchmarking activities.
  • Utilize PDSA methodology and analytical tools to evaluate performance improvement initiatives.
  • Oversee risk management systems to identify and mitigate high-risk processes in care delivery.
  • Ensure effective data reporting and analysis to support strategic decision-making and quality improvement.
  • Collaborate with the Patient Experience team to enhance service quality and patient satisfaction.
  • Manage performance of staff, providing training, development, and corrective action as needed.
  • Participate in budget preparation and monitor productivity throughout the year.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; Master’s degree in nursing or related field preferred.
  • Registered Professional Nurse licensed in New York State.
  • Minimum 5 years of experience in quality management or performance improvement.
  • Proficiency in statistical process control tools and data analysis methodologies.
  • Knowledge of Federal and NYS Department of Health regulations.
  • Experience with CMS Hospice Quality Reporting Program and performance improvement projects.
  • Strong leadership and collaboration skills to manage teams and drive quality initiatives.
  • Ability to utilize corporate resources effectively, including HR, Compliance, and Information Services.
