Overview
- Lead quality management initiatives for hospice and palliative care services, ensuring compliance with regulatory standards and enhancing patient care quality.
- Develop and implement Quality Assurance/Performance Improvement (QAPI) plans aligned with industry standards and patient needs.
- Collaborate with senior leadership to recommend strategic quality initiatives and report on key quality indicators.
- Utilize data analysis tools to monitor clinical performance and identify improvement opportunities.
- Ensure compliance with Federal and NYS regulations, serving as a resource on regulatory matters.
- Oversee CMS Hospice Quality Reporting Program, implementing performance improvement projects based on data insights.
- Manage staffing and resources to support departmental services, including recruitment and performance management.
- Facilitate collaboration across clinical and operational departments to enhance care processes and outcomes.
Key Responsibilities & Duties
- Develop and maintain quality management programs for hospice services, ensuring alignment with patient needs and regulatory requirements.
- Collaborate with leadership to establish clinical performance indicators and manage benchmarking activities.
- Utilize PDSA methodology and analytical tools to evaluate performance improvement initiatives.
- Oversee risk management systems to identify and mitigate high-risk processes in care delivery.
- Ensure effective data reporting and analysis to support strategic decision-making and quality improvement.
- Collaborate with the Patient Experience team to enhance service quality and patient satisfaction.
- Manage performance of staff, providing training, development, and corrective action as needed.
- Participate in budget preparation and monitor productivity throughout the year.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; Master’s degree in nursing or related field preferred.
- Registered Professional Nurse licensed in New York State.
- Minimum 5 years of experience in quality management or performance improvement.
- Proficiency in statistical process control tools and data analysis methodologies.
- Knowledge of Federal and NYS Department of Health regulations.
- Experience with CMS Hospice Quality Reporting Program and performance improvement projects.
- Strong leadership and collaboration skills to manage teams and drive quality initiatives.
- Ability to utilize corporate resources effectively, including HR, Compliance, and Information Services.
