Job Description

Overview

  • Deliver compassionate and personalized nursing care to hospice patients, addressing their physical, emotional, and psychosocial needs.
  • Collaborate with interdisciplinary teams to develop and implement holistic care plans tailored to individual patients.
  • Provide education and emotional support to patients and families during challenging end-of-life transitions.
  • Supervise and guide home health aides and personal care workers to ensure high-quality patient care.
  • Conduct detailed assessments to evaluate patients' physical and emotional conditions and adjust care plans as needed.
  • Maintain accurate and timely clinical documentation to support seamless care delivery.
  • Coordinate with physicians to manage and update patient care plans effectively.
  • Contribute to a supportive and collaborative work environment focused on patient-centered care.

Key Responsibilities & Duties

  • Perform comprehensive physical and psychosocial assessments of hospice patients to determine their care needs.
  • Develop and implement individualized nursing care plans that prioritize patient comfort and dignity.
  • Provide direct nursing care and support to patients in their homes, ensuring optimal comfort.
  • Supervise and evaluate the performance of home health aides and personal care workers.
  • Educate patients and families about hospice care processes and available resources.
  • Collaborate with physicians and interdisciplinary teams to adjust care plans and achieve desired outcomes.
  • Maintain detailed and accurate clinical documentation in patient records.
  • Participate in team meetings to ensure coordinated and effective patient care delivery.

Job Requirements

  • Associate of Science in Nursing (AS) or equivalent educational background required.
  • Minimum of 2 years of clinical experience, preferably in hospice or acute care settings.
  • Strong proficiency in clinical documentation and data entry systems.
  • Excellent communication and interpersonal skills to support patients and families effectively.
  • Ability to work collaboratively within interdisciplinary teams to deliver patient-centered care.
  • Experience supervising and mentoring home health aides and personal care workers.
  • Preferred experience in hospice care; home care and acute care experience considered.
  • Commitment to providing compassionate and dignified care to patients at the end of life.
