Job Description

Overview

  • Provide compassionate care as a Hospice Nurse, delivering essential services to patients in their homes and communities.
  • Coordinate and manage individualized care plans, ensuring optimal outcomes for patients and families.
  • Utilize nursing expertise to perform assessments, administer treatments, and educate patients on health management.
  • Collaborate with physicians and healthcare teams to address changing patient needs effectively.
  • Engage in community health education to enhance patient care and support.
  • Maintain compliance with state nursing practice regulations and organizational policies.
  • Contribute to staff meetings and professional development initiatives to improve care delivery.
  • Document patient care activities accurately and timely using prescribed tools.

Key Responsibilities & Duties

  • Conduct initial and ongoing patient assessments to determine care needs and plan interventions.
  • Develop and revise care plans based on patient status and physician directives.
  • Administer medications and treatments in accordance with physician orders and nursing standards.
  • Provide health education to patients and families to support care plan objectives.
  • Coordinate care with multidisciplinary teams to ensure comprehensive patient support.
  • Maintain accurate and timely documentation of patient care activities and communications.
  • Participate in staff meetings and contribute to team discussions and initiatives.
  • Ensure compliance with organizational policies and state nursing regulations.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution required.
  • Current RN licensure and CPR certification required; valid driver’s license necessary.
  • Minimum 1 year of acute care or home care nursing experience; 5 years preferred.
  • Proficiency in nursing skills, including patient assessment and care plan development.
  • Ability to supervise home health aides and coordinate multidisciplinary care.
  • Strong communication, problem-solving, and documentation skills essential.
  • Physical capability for prolonged standing, walking, and lifting required.
  • Experience driving in various weather conditions to reach patient locations.
