Overview
- Provide leadership and clinical oversight in a residential treatment facility for youth aged 12-18 with complex mental health diagnoses.
- Collaborate with interdisciplinary teams to ensure effective therapeutic and educational support for residents.
- Develop and implement transitional and permanency programs to support successful reintegration into the community.
- Supervise and mentor a team of clinical and support staff to ensure high-quality care and compliance with regulations.
- Engage families in the treatment process to strengthen relationships and improve outcomes for youth.
- Ensure compliance with all relevant guidelines and standards, including OMH regulations.
- Participate in administrative meetings and provide updates on program progress and outcomes.
- Maintain collaborative relationships with local resources and referral systems to enhance support for youth and families.

Key Responsibilities & Duties
- Oversee the development and implementation of transitional programs to reduce length of stay and prevent re-referrals.
- Provide clinical supervision to teams, focusing on family dynamics, educational success, and socialization needs.
- Coordinate discharge planning and ensure smooth transitions for residents into community settings.
- Establish collaborative relationships with community-based programs to support families effectively.
- Participate in clinical and treatment meetings, presenting updates and progress reports.
- Supervise documentation and compliance with best practice standards and policies.
- Manage transitional and family support teams, ensuring effective coordination and evaluation of services.
- Provide direct transitional coordinator services as needed and address team vacancies.
- Support staff training in Therapeutic Crisis Intervention and verbal de-escalation techniques.
Job Requirements
- Master's degree in Social Work or a related human services field.
- LCSW certification with at least 2 years of direct service experience with children with SED.
- Minimum of 2 years of supervisory or managerial experience in the social service sector.
- Experience in residential settings and working with youth with mental health challenges.
- Familiarity with housing, ACS, foster care systems, and electronic health records.
- Ability to conduct home visits and navigate community resources effectively.
- Strong organizational, communication, and interpersonal skills.
