Overview

  • Deliver exceptional nursing care and case management for individuals with developmental disabilities in a supportive remote environment.
  • Collaborate with interdisciplinary teams to ensure optimal health outcomes and regulatory compliance.
  • Develop and implement individualized nursing care plans, adapting them to changing health conditions.
  • Conduct regular health assessments and monitor patient progress, ensuring timely interventions.
  • Utilize advanced electronic health record systems to streamline patient care coordination.
  • Train and supervise staff on nursing protocols, health measures, and medication administration.
  • Participate in interdisciplinary meetings to align care strategies and enhance patient outcomes.
  • Ensure adherence to state and federal regulations in all nursing practices and documentation.

Key Responsibilities & Duties

  • Provide direct nursing care and perform physical examinations upon program entry and quarterly thereafter.
  • Coordinate acute and ongoing healthcare services tailored to individual needs and regulatory standards.
  • Monitor patient health statuses, lab results, and treatment efficacy, ensuring accurate documentation.
  • Train staff on health and hygiene practices, communicable disease control, and client-specific medical protocols.
  • Supervise LPNs and direct support professionals, conducting performance evaluations and maintaining readiness.
  • Participate in hiring and scheduling of nursing staff to ensure optimal team functionality.
  • Provide on-call support to ensure continuity of care and emergency preparedness.
  • Maintain compliance with all regulatory standards in nursing practices and documentation.

Job Requirements

  • Associate of Occupational Studies (AOS) degree required, with relevant nursing certifications preferred.
  • Minimum of 1 year of nursing experience, ideally with developmental disabilities populations.
  • Proficiency in electronic health record systems and care coordination tools.
  • Strong knowledge of regulatory compliance standards in healthcare settings.
  • Ability to train and supervise staff in nursing protocols and health measures.
  • Excellent communication skills for effective interdisciplinary collaboration.
  • Flexibility to provide on-call support and adapt to dynamic healthcare needs.
