Overview
- Lead and mentor a team of nursing professionals in hospice care.
- Ensure consistent delivery of hospice and palliative care services aligned with program goals.
- Collaborate with interdisciplinary teams to optimize patient and family care outcomes.
- Maintain compliance with state and federal regulations in documentation and care practices.
- Provide leadership in scheduling and staffing to meet organizational standards.
- Engage in community outreach and presentations to promote hospice services.
- Support the Associate Director in operational and strategic initiatives.
- Foster excellence in pain and symptom management among nursing staff.
- Maintain availability for incident follow-ups beyond core hours.
Key Responsibilities & Duties
- Supervise and mentor hospice nursing staff to ensure high-quality care delivery.
- Coordinate staff assignments in compliance with established ratios and policies.
- Monitor nursing documentation for accuracy and regulatory compliance.
- Empower RN Case Managers in achieving patient-centered care goals.
- Collaborate with outreach departments for community presentations.
- Ensure availability for on-call support and incident management.
- Implement strategies for effective pain and symptom management.
- Support interdisciplinary team collaboration for holistic patient care.
- Assist in operational planning and execution with the Associate Director.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of five years of hospice nursing experience required.
- Supervisory or leadership experience preferred.
- New York State RN licensure mandatory.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred.
- Proficiency in Word, Outlook, and Windows applications.
- Strong ability to supervise and mentor nursing staff effectively.
- Experience in triage, on-call, or after-hours care is essential.
- Availability to manage incidents outside regular working hours.
