Overview
- Serve as the primary liaison between members and the care plan, ensuring effective communication and coordination of services.
- Develop and review person-centered service plans, addressing members' needs and preferences comprehensively.
- Coordinate benefits and services, including covered and non-covered options, for optimal member support.
- Maintain regular monthly contact with members to ensure ongoing support and follow-up on identified needs.
- Document member interactions and care plans thoroughly and accurately in compliance with organizational standards.
- Participate in weekly team meetings to discuss member cases and collaborate on care strategies.
- Work onsite three days a week during the initial training period, transitioning to a hybrid schedule thereafter.
- Utilize bilingual skills in Chinese, Spanish, or Bengali to effectively communicate with diverse members.
Key Responsibilities & Duties
- Conduct member assessments to identify needs and develop tailored care plans.
- Coordinate with family members and vendor agencies to ensure comprehensive care delivery.
- Document interactions and care plans in member charts promptly and accurately.
- Participate in team meetings to discuss member progress and share insights.
- Utilize Excel for basic data management tasks, including sorting, filtering, and saving documents.
- Provide bilingual support in Chinese, Spanish, or Bengali to enhance member communication.
- Ensure compliance with clinical review procedures and organizational standards.
- Maintain a hybrid work schedule, balancing onsite and remote responsibilities effectively.
Job Requirements
- Associate of Applied Science (AAS) degree required; MSW or RN preferred.
- Minimum of 1 year of case management experience, with bilingual proficiency in Chinese, Spanish, or Bengali.
- Proficiency in basic Excel tasks, including sorting, filtering, and document management.
- Strong knowledge of member assessment and clinical review procedures.
- Ability to communicate effectively with members across diverse socioeconomic backgrounds.
- Experience in maintaining accurate and thorough documentation of member care plans.
- Willingness to work onsite during the initial training period and transition to a hybrid schedule.
