Job Description

Overview

  • Lead and manage the case management department in a dynamic healthcare environment, ensuring compliance with organizational policies and regulatory standards.
  • Coordinate patient care and discharge planning processes across hospital departments and external agencies for seamless care transitions.
  • Oversee daily operations of hospital case management, driving performance and quality improvement initiatives.
  • Engage interdisciplinary teams and external stakeholders to achieve established outcomes and optimize clinical resource utilization.
  • Demonstrate executive presence with excellent communication, presentation, and motivational skills to lead the department effectively.
  • Utilize leadership and change management skills to implement case management initiatives successfully.
  • Adapt quickly to changes and manage multiple priorities with minimal supervision.
  • Collaborate with hospital leadership to align department goals with organizational objectives.

Key Responsibilities & Duties

  • Direct and evaluate departmental operations, including staffing, onboarding, and competency development to meet performance objectives.
  • Allocate resources effectively to achieve productivity and quality goals within the department.
  • Ensure adherence to care coordination and discharge planning processes with accurate documentation.
  • Collaborate with financial and case management leaders to achieve organizational goals.
  • Monitor and improve department performance through data analysis and sustainable improvement activities.
  • Establish relationships with key stakeholders, including medical and ancillary service leaders.
  • Maintain compliance with policies, standards, and codes of conduct.
  • Oversee case management, social work, and chaplain departments to ensure integrated operations.

Job Requirements

  • Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
  • Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required.
  • Minimum of 3 years of acute care hospital case management experience required; 5 years preferred.
  • At least 2 years of experience in case management leadership required.
  • Strong strategic relationships with physicians and post-acute community essential.
  • Proven ability to review data, develop action plans, and lead performance improvement initiatives.
  • Exceptional communication, presentation, and motivational skills required.
  • Self-motivated and capable of managing multiple priorities independently.
