Overview
- Provide medical case management and care coordination for residents in permanent supportive housing.
- Support individuals with complex medical and behavioral health needs to achieve independence and improved health outcomes.
- Collaborate with healthcare providers, pharmacies, and community resources to ensure comprehensive care.
- Conduct health assessments, monitor wellness, and maintain accurate medical documentation.
- Assist clients with daily needs, including navigating bills and accessing community services.
- Participate in interdisciplinary treatment team meetings and case conferences.
- Provide health education and advocate for clients' healthcare goals and preferences.
- Respond to emergencies with First Aid and CPR as necessary.
- Supervise assigned staff and contribute to program development.
Key Responsibilities & Duties
- Serve as the primary medical case manager for assigned clients, ensuring their healthcare needs are met.
- Conduct regular health assessments and monitor clients' wellness through vital signs checks.
- Coordinate care with physicians, pharmacies, and community providers to optimize treatment plans.
- Maintain detailed medical documentation, including medication administration records and controlled substance logs.
- Participate in interdisciplinary meetings to develop and review client treatment plans.
- Educate clients on health topics and advocate for their healthcare preferences.
- Provide emergency care, including First Aid and CPR, as required.
- Assist clients with daily tasks, such as managing bills and accessing community resources.
- Supervise staff and support program operations as needed.
Job Requirements
- Active New York State Licensed Practical Nurse (LPN) license.
- Minimum of 1 year of experience, preferably 3 years, in psych/shelter populations.
- Associate of Applied Science (AAS) in Nursing or related field.
- Proficiency in medical documentation and care coordination.
- Strong knowledge of community healthcare resources and social services.
- Ability to perform First Aid and CPR in emergency situations.
- Excellent organizational and communication skills.
- Experience supervising staff and contributing to team operations.
