Overview
- Provide compassionate, patient-centered care to individuals in hospice, ensuring dignity and comfort during end-of-life stages.
- Conduct comprehensive assessments and develop personalized care plans addressing physical, emotional, and psychosocial needs.
- Collaborate with interdisciplinary teams to deliver holistic and coordinated care for patients and their families.
- Educate patients and families about hospice services, care processes, and available support resources.
- Supervise and mentor home health aides, ensuring adherence to quality standards and best practices.
- Maintain meticulous documentation of patient care activities, clinical data, and progress updates.
- Ensure compliance with hospice care policies, standards, and regulatory requirements at all times.
- Provide emotional support to patients and families, fostering a compassionate and supportive environment.
Key Responsibilities & Duties
- Conduct initial and ongoing assessments to evaluate hospice patient care needs and conditions.
- Develop and implement individualized nursing care plans tailored to patient health status and preferences.
- Coordinate with physicians and interdisciplinary teams to ensure seamless communication and care delivery.
- Monitor patient progress and adapt care plans to address evolving needs and conditions effectively.
- Provide education and emotional support to patients and families regarding hospice care and coping strategies.
- Supervise and train home health aides and personal care workers to uphold quality care standards.
- Ensure accurate and timely documentation of patient care activities and clinical observations.
- Maintain compliance with hospice care standards, policies, and regulatory requirements consistently.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or related field required.
- Minimum of 1 year of clinical nursing experience, preferably in hospice or acute care settings.
- Preferred 6 years of experience in hospice or related nursing roles.
- Strong ability to collaborate effectively within interdisciplinary team environments.
- Proficiency in clinical data input and maintaining accurate documentation.
- Excellent communication, teaching, and listening skills to support patients and families.
- Knowledge of hospice care standards, practices, and regulatory requirements preferred.
