Overview

  • Deliver exceptional nursing care to patients in their homes, ensuring adherence to individualized care plans and promoting optimal health outcomes.
  • Conduct comprehensive physical assessments and provide skilled nursing interventions tailored to diverse patient needs.
  • Collaborate with interdisciplinary teams to develop and implement effective preventive and rehabilitative care strategies.
  • Educate patients and families on health management, fostering understanding and compliance with care plans.
  • Maintain detailed and accurate documentation of all patient interactions and care activities.
  • Participate in care planning meetings to align patient needs with healthcare objectives.
  • Provide compassionate care to individuals with complex conditions, ensuring dignity and comfort.
  • Support and mentor aides to uphold high standards of patient care and service delivery.

Key Responsibilities & Duties

  • Perform thorough physical assessments and document findings to guide patient care plans effectively.
  • Monitor and reassess patient conditions, ensuring consistent delivery of skilled nursing care.
  • Implement preventive and rehabilitative procedures tailored to individual patient needs and goals.
  • Communicate changes in patient conditions promptly to healthcare providers for timely interventions.
  • Educate patients and families on managing health conditions and adhering to treatment plans.
  • Actively participate in interdisciplinary care planning conferences to optimize patient outcomes.
  • Complete all required documentation accurately and within established timelines.
  • Conduct supervision visits and assist in training aides to ensure quality care delivery.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent educational background.
  • Active Registered Nurse (RN) license with a minimum of 1 year of field nursing experience.
  • Experience working with patients with complex conditions such as dementia or tuberculosis preferred.
  • Strong documentation skills with the ability to meet deadlines consistently.
  • Effective communication and counseling skills to educate patients and families.
  • Proficiency in patient-centered care planning and interdisciplinary collaboration.
  • Comfortable working in the field and traveling within designated areas.
  • Preferred experience in LCSA or similar healthcare settings.
