Job Description

Overview

  • Provide in-home therapy services to enhance communication and swallowing functions.
  • Develop and implement individualized treatment plans based on comprehensive evaluations.
  • Ensure compliance with physician orders, organizational policies, and regulatory standards.
  • Focus on patient-centered care and education tailored to individual needs.
  • Collaborate with interdisciplinary team members to coordinate care and referrals.
  • Maintain accurate and timely documentation in electronic medical records (EMR).
  • Monitor patient progress and adjust treatment plans as needed.
  • Adhere to organizational competencies and professional standards.

Key Responsibilities & Duties

  • Conduct comprehensive evaluations in the home setting within required timeframes.
  • Deliver skilled speech-language pathology services per physician’s plan of care.
  • Educate patients and caregivers on communication strategies and home programs.
  • Collaborate with physicians and team members for effective care coordination.
  • Participate in discharge planning throughout the course of care.
  • Maintain productivity standards in a home health setting.
  • Document all case communication securely and accurately.
  • Demonstrate flexibility and perform additional duties as assigned.

Job Requirements

  • Master’s Degree in Speech-Language Pathology is required.
  • Active Speech Language Pathologist License (NJ) is mandatory.
  • AHA Basic Life Support (BLS) Certification is essential.
  • Minimum of 6 months experience in Speech Language Pathology.
  • Proficiency in basic computer operations and electronic medical records (EMR).
  • Ability to lift, push, and pull up to 50 lbs.
  • Capability to stand for extended periods (up to 8 hours/day).
